Fees and Insurance
Fees
My fee is typically $165 per 50 minutes. Your first intake session is $200. In addition, there may be charges for specialized assessments, written documents such as certain kinds of letters, reports, treatment plans, case management services and/or telephone calls. I will inform you of these when applicable prior to provision of service. If you are paying out-of-pocket, please inquire about sliding scale opportunities.
Insurance
I am happy to bill your insurance company for services. The insurance providers that I'm contracted with are Regence/Blue Cross-Blue Shield, Pacific Source and Moda. For all other insurance companies, I am an out-of-area provider. This means that the co-pay is typically higher. I am not a contracted provider for Medicare/OHP.
Any unpaid fees are your responsibility. If you have questions about third party billing, or need some assistance, please discuss this with me.
New Client Forms
If you are a new client, you will need to download, complete and bring with you my client forms. (It will take you about 30 minutes to complete the forms.) New client forms can be found here.
Payments
Fees, including any co-payment, are due in full at each session, unless we specifically agree to other arrangements. I accept cash, checks, or credit cards through Paypal.
